  1. Contact us for initial screening, by phone or in person.

  2. Finalize financial details and insurance with our program staff. The following procedures and tests (see below) must be done no earlier than 2 weeks prior to admission. We request that we have the results of your procedures a week before your start date. Our physician will then determine whether your physical condition is medically appropriate for the lifestyle demands of our program.

  3. Take the following list of procedures and tests to your doctor or lab to ensure that all requirements are fulfilled. Request that these procedures are done STAT and ask for the results to be faxed to our office at 850-893-6994.

  4. Establish admission date with program staff. Entrance into the program is mutually decided on and is based on eating disorder screening, medical appropriateness and financial elements.

  5. Complete a telephone intake interview.

This patient is preparing for immediate admission into our program and requires the following procedures and tests STAT. Please FAX all results, as they become available, to Canopy Cove. Our FAX number is 850-893-6994. The patient will not be eligible for admission unless these results reach our office and are reviewed by our physician prior to the scheduled admission date.
